Dr. John Baer, Dr. Howard Hartzell and the staff at South Penn Eye Care want to make sure that we have the information we need to take the best possible care of your eyes.  Please review the following before your visit.

Preparing for your Visit

Please bring the following to your visit: 

  A list of current medications, including eye medications
  A list of your current medical problems
  Your current glasses and sunglasses
  The name and address of the physician or optometrist who referred you
  The name and address of your family doctor
  Your insurance cards and referral forms. If you are insured by an HMO, please have the necessary authorization and referral forms before your appointment

Dilation of your pupil
We may need to dilate your pupils at your exam.  Dilation assesses the retina, optic nerve and blood vessels in the back of your eye.
It may blur your vision and make you sensitive to light for several hours, so please bring sunglasses with you.
You may wish to have a friend or relative drive you home because dilation may blur your vision for driving.
